Department of Labor SOII Forms

The so-called Survey of Occupational Injuries and Illnesses, short SOII is a survey of the United States Department of Labor statistics bureau that is required to be filed every year. All required forms can be directly downloaded at the internet address http://www.bls.gov/respondents/iif/forms.htm. The results from these forms are needed to provide an estimate of the number of work related illnesses and injuries and their frequency.

If returning the form is mandatory depends on state law and is regulated by the OSHA for all private employers. Only a fraction os the actual establishments are participating in the survey and numbers from this sample size are used to create statistics.
