Passport forms are required for a passport application in the United States. While there is a possibility to use a special online wizard at http://travel.state.gov/passport/forms/forms_847.html you can also download the forms directly and fill them by hand. In some cases additional passport affidavits often referred to as sworn statements are required as well. They are not a standard part of the application and normally not required for citizens of the United States of America.
For reading and filling the forms you need to install the Adobe Acrobat reader or a similar software like Foxit on your computer. Please note that there are guidelines for filling these forms that should be followed to ensure acceptance. They must be clearly legible and filled with black ink on matte white paper. The so-called Survey of Occupational Injuries and Illnesses, short SOII is a survey of the United States Department of Labor statistics bureau that is required to be filed every year. All required forms can be directly downloaded at the internet address http://www.bls.gov/respondents/iif/forms.htm. The results from these forms are needed to provide an estimate of the number of work related illnesses and injuries and their frequency.
If returning the form is mandatory depends on state law and is regulated by the OSHA for all private employers. India requires filing income tax forms corresponding to the type of income you are earning in India. This includes salaries, pensions as well as income from property, business owners and more.
